Perinatal Program

The Perinatal Program provides prevention services to pregnant and postpartum women struggling with substance use.

Motherhood and Maternity Addiction Services (MAMA’s), provide wraparound services to pregnant women receiving substance abuse treatment at Sojourner Recovery Services.

Our vision is to save lives, foster hope, and strengthen communities.

Perinatal Services Include:

We provide referrals for perinatal, pediatric, and primary care.

This can include:

  • substance use treatment
  • mental health counseling
  • nutrition counseling
  • parenting classes

We provide clear communication and education about the effects of maternal substance use and its risks to the fetus.

For individuals living with both substance use and mental health concerns, MAT offers a supportive bridge to healing. Though primarily offered through Sojourner, MAT is often coordinated with TLC’s behavioral health and psychiatric services.

Physical health and mental health are deeply connected. Our integrated medical services support everything from routine care to medication monitoring—ensuring you feel well in every way.

Sometimes healing needs extra structure. Transitional Living (TLC) offers 24/7 residential mental health programs that provide a safe place to stabilize, build daily living skills, and work toward independence.

Case management helps people with mental illness manage their symptoms, stay connected to care, and meet personal goals while living in the community.

Need help keeping everything organized? Our case managers assist with benefits, housing, appointments, and anything else that helps you stay on track.

We help individuals overcome barriers to employment and schooling—offering support with training, applications, and confidence building along the way.

TLC partners with local courts, jails, and community agencies to provide mental health support for individuals navigating the justice system, including jail-based services and discharge planning.

We offer integrated care through coordinated services between TLC and Sojourner, ensuring you receive support for both sides of your experience.

We provide tailored care for people experiencing homelessness, those reentering the community after incarceration, and more. Whether your need is clinical, social, or practical—we meet you where you are.

Employment Services are offered from Onio Means Jobs

A dedicated case manager assists with creating and implementing an individualized self-sufficiency case plan.  This includes continued recovery, posite and effective parenting, self-care, employment, and permanent housing options.

Experiencing intense mood swings that impact your functioning or relationships.
If symptoms arise, we’ll work with our psychiatric team to support mood stabilization and coping.

Recurring thoughts or compulsions that interfere with daily life.
We’ll help you explore treatment approaches that reduce distress and restore balance.

Difficulty falling asleep, staying asleep, or managing disruptive dreams.
We’ll explore emotional, environmental, and physical causes—then work together toward better rest.

A sense of being frozen, disconnected, or emotionally exhausted.

Stress from changes like divorce, job loss, relocation, or becoming a parent.
We’ll help you adapt, cope, and stay grounded through uncertain times.

Unhealthy thoughts or behaviors around eating or self-image.

Experiencing control, fear, or harm in a relationship.
We’ll listen without judgment, prioritize your safety and confidentiality, and help connect you with the right supports.

Sadness or emptiness after losing someone or something important.
If you’re grieving, we’ll help you process at your own pace, in a way that feels authentic and safe.

Ongoing pain from past events that still affect you today.
Our trauma-informed providers will help you rebuild trust in yourself and others—with support that honors your pace.

Racing heart, tight chest, or sudden feelings of dread.
We’ll explore both physical and emotional causes and offer calming tools that help you feel grounded and safe.

Conflict, distance, or confusion in key relationships.
We’ll help you explore communication patterns, boundaries, and tools for connection.

Wanting to quit tobacco or vaping but needing support to do so.
We’ll talk through motivation, withdrawal tools, and strategies that match your readiness.

Struggling with both mental illness and addiction.
We offer integrated care through coordinated services between TLC and Sojourner, ensuring you receive support for both sides of your experience.

Feeling cut off from others or unsure where to turn.
We’ll help you reconnect—with peer support, group services, or just someone to listen.

Our team can help you navigate barriers, build stability, and stay focused on what matters to you.

Using substances in ways that feel harmful or hard to stop.
If substance use is present alongside mental health concerns, we’ll coordinate care with Sojourner and offer behavioral health support that addresses the whole picture.
